NEW PATIENT CONSULTATION
EXTENDED LAB PANEL AND PHYSICIAN IN-PERSON CONSULTATION

The Original T Package
Testosterone. Management. Labs/Physician Visit every 3 months
Testosterone sent directly to your house. You get quarterly physician visits*. Surveillance labs every 3 months to ensure adequate treatment and monitor for potential complications. If any additional medications such as anastrazole, sildenafil, diuretics, etc are needed they can be dispensed immediately on site. Most meds are $15 a month.
*visits can be virtual, in-house/offsite, or office based.
- We do require at least one in-person visit every 6 months. If you opt to do virtual visits we will order labs ahead of time so you can have them drawn at Lab Corp. This allows us to review results at your visit. If in person we will draw your blood at that time and call to discuss results.
- Any additional visits or labs that are required are at normal office visit and lab prices.

Testosterone. Management. UNLIMITED Physician VisiTS. QUARTERLY LABS
CONCIERGE STYLE PLAN. UNLIMITED VISITS. IN-HOME/WORK, VIRTUAL, IN-OFFICE. ED MEDICATIONS, ANASTROZOLE AND MANAGEMENT INCLUDED. QUARTERLY LABS INCLUDED.
